In order to help Dr. Bolfek better understand your child needs (or your own needs) you will be asked to fill up the questionnaire and email it back at least 48 hours before the scheduled visit.
Dr. Bolfek will review all your responses and will be fully prepared to do detailed assessment even before she had a pleasure meeting your child or you.

Initial appointment is comprehensive assessment and is 60-75 minutes long – 50 minutes for assessment and 10-15 minutes for feedback and detailed treatment plan.

Follow up appointments are 25-45 minutes long and are very individualized to fit your or your child needs.
